1. Name of Activity/Project : Social preparation for the establishment of Stimulation & Therapeutic Activity Center (STAC) in the province of Pangasinan. Period : July 2003 to March 2004 Your Responsibility : Coordinator and facilitator How did you utilize it :

   Coordinated with the Regional, Provincial, City & Municipal government units for the establishment of STAC in the province. Facilitated project orientation on different government levels to officials, staffs of department heads concerning the welfare of person’s with disabilities.

2. Name of Activity / Project : Establishment of STACenters and its satellites. Period ： April 2004 to present Your Responsibility : Center-based field staff, facilitator and coordinator How did you utilize it :

   Conducts physical rehabilitation to children with disabilities, facilitates meetings and orientation of project & coordinates linkages with the local government units, NGO’s private groups, etc.

Describe your needs and expectations for the follow-up program.

The needs and expectations for the follow up program are information/journals on Vocational Rehabilitation or work center; information on the development of each participant’s future plan made and visit of JSRPD staff or expert to assist and help on the need of each participant’s plan and give suggestions for improvement.

Describe your future plans in the disability field if any.
To establish a special school for children with disabilities and conduct feasible study on

how to start and manage or oversee a work center for person’s with disabilities thru linkages with the National and local government units, department heads, NGO and international funding agencies.
